When patients are experiencing substance use disorder in hospitals, we help them assess their needs, create a personalized care plan, and find treatment options to support them. The Hospital Recovery Connection program creates a seamless connection between hospital teams and substance use care providers. This program is made possible by a state grant, which refers to the program as the Statewide Opioid Response (SOR) Hospital Screening and Warm Handoff. By meeting individuals at a critical moment, the program makes substance use disorder treatment more accessible and supports lasting recovery.
